    Hello Everyone,

    I am in Access Services at the Lyman Beecher Library at Norfolk State University.    I am pulling reserve items that need to be returned to professors.  Does anyone have a template that I could use for a form for professors to sign when books are returned to them? It is important that the library has paperwork to verify that reserve items have been returned.
